LAST REGISTRATION EVENT FOR THE 2010 SEASON

The following forms will be needed in order to register your child for the 2010 season with the Lumberton Youth Football and Cheerleading league. Please print out all forms, fill out completely, and bring to the next scheduled registration time.
In addition to the above forms you MUST also bring:
COPY of childs BIRTH CERTIFICATE
COPY of childs recent REPORT CARD
(for 5 year olds not yet in school, please bring COPY of recent UTILITY BILL)

Payment for any/all fees due

Physical Exams will be offered at TRYOUTS for $5.00 each.
A physical is REQUIRED for your child to participate.
If you prefer to take your child to your own RN or Physician
